    Pet Release Form

  • AUSSIE PET MOBILE’S philosophy is to offer a stress-free grooming experience for your loved pet. We pay special attention to older pets and pets with health issues. Your pet’s safety and comfort are our main priority. We will not perform any grooming procedure that causes pain or a level of stress that we think is excessive. This includes nail trimming in some situations. Senior pets will be groomed for cleanliness and comfort.

    MEDICAL / VACCINATIONS:

You the pet owner will advise us of any medical, physical, emotional issues, allergies, sensitivities or pre-existing conditions. These may include prior surgeries, hip or joint issues, warts, moles, ear infections or skin problems.

AUSSIE PET MOBILE® requires your pet to have its current rabies vaccination for every pet being groomed.  While AUSSIE PET MOBILE® adheres to a stringent sanitation procedure for all trailers and equipment, we also strongly advise that each pet be vaccinated against distemper, hepatitis, parainfluenza adenovirus type 2, parvovirus, corona, leptospirosis, and bordetella prior to being groomed. You the pet owner acknowledge that your pet is current for all required vaccinations, and that you can provide proof of such vaccinations on request. We will require the name and contact info for your current veterinarian.

    BEHAVIORAL CHALLENGES:

AUSSIE PET MOBILE® has the right to refuse grooming any animal that may be a threat to itself or any person.  If we are not able to take a cat out of its carrier or get a muzzle on a dog that may bite, then we may have to decline services for its own protection and ours.  The pet may get away or get injured if it is undergoing a great amount of stress. If we arrive at your property and your pet cannot be groomed for any reason a minimum $50.00 groom fee will apply. The animal owner will be held responsible for any bites that may require medical treatment and for any property damage caused by their pet. All bites will be reported to the local authorities as required by law.  AUSSIE PET MOBILE® does not use any type of tranquilizers on any pet. We do not cage pets for any reason.

AUTHORIZATION FOR SHAVING A PET AND/OR FLEA & TICK TREATMENT:

Shaving a pet often exposes pre-existing skin conditions.  If an animal is badly matted, you may find sores and tender skin under those mats.  Many times the skin is so sore that the clippers may cut the skin, especially while taking mats off of a cat that has very thin skin.  There is no way to avoid this, however slow or careful we are.

In order to protect transfer of fleas from one appointment to the next, it is our policy to reschedule appointments immediately following a pet with fleas, to flea bomb the van and tools, and for the groomer to change uniforms. If you suspect your pet has fleas or ticks, please inform us prior to the service so that we can schedule your appointment as the last one for the day. If fleas or ticks are found during the grooming process, your pet will be given a treatment for such and you will be charged an additional fee.

    APPOINTMENTS:

We will always arrive as close to the scheduled appointment window as possible, however, sometimes situations occur that will disrupt the daily grooming schedule. This includes traffic, weather, and mechanical difficulties, spending more time with an older/younger or difficult pet. We will never rush through service in order to stay on schedule as that can compromise a pet’s safety. You will be called and advised of an arrival time when schedules change beyond our control.

For your first appointment, you must be home in order to introduce us to your pet. On subsequent visits we are happy to retrieve your pet from your yard, garage, or home depending on what arrangements have been made with us and provided it can be done safely. It is not our policy to keep housekeys belonging to our customers. Cats must be left in a carrier for their safety and ease in retrieval. We have a $50 minimum for all service visits.

    CANCELLATION / NO SHOW POLICY:

Missed appointment and cancellation impact our service schedule and impact our groomer’s earnings. Out of fairness to them and our other customers we require 48 hours notice to change or cancel an appointment. We will send automatic reminders prior to your appointment. Providing us advance notice gives us the opportunity to fill your grooming appointment and to rearrange our schedule to accommodate other clients. Failure to provide sufficient notice will result in a late cancellation fee of $40 that must be paid before any subsequent appointments are booked.

If we show up for a scheduled and confirmed appointment and there is no one home to release the pet, a $40.00 ‘no show’ fee will apply. This must be paid before any subsequent appointments are booked. This policy will also apply if we are unable to safely retrieve your pet from your property for a scheduled appointment.

    PRICING:

Pricing varies according to each pet and is determined by breed, size and the condition of your pet. Prior to your appointment, we will confirm the services and the price with you. Additional charges apply for other services provided which may include but not be limited to: de-matting, de-shedding, clipping, excessive coat and flea treatments due to the additional time and materials it takes to safely complete such services. A gratuity of 10-20% of the cost of services will be much appreciated by your groomer. Payment in full is expected at the time the services are concluded and your pet is returned; we accept cash, checks and credit cards. Payment by credit card or check, and a cash gratuity, is encouraged. Groomers are unable to provide change. A $25 fee will be applied for any checks returned by the bank for insufficient funds.

    MATTING AND EXCESSIVE UNDERCOATS:

Unmaintained coats cause a variety of health problems for pets including itching and skin infections. Fleas, ticks and other parasites may be lurking in the coat causing further skin infections. The skin underneath can be raw and inflamed. Severely matted coats will require a shave down. We are happy to show you how to care for your pet’s coat in the future, however we cannot brush out a coat that is already severely matted due to the stress and discomfort it will cause your pet. During the shave down your pet may be nicked or cut because we have to work so closely to the skin to remove the coat and your pet may have warts, moles, or skin folds hidden in the mats. We are very careful, but it is possible an injury may occur. We will always ask for your consent prior to providing a shave down; if you do not consent, your pet will be returned to you without grooming services being provided.

    ANAL GLANDS EXPRESSION:

Our policy is NEVER to express anal glands due to the potential problems this may cause, ranging from causing the pet to become reliant on gland expression, to irritation, infection, or even serious injury. If the groomer notices a likely blockage or other issue, they will inform you; but it is our corporate policy that such procedures are best and most safely conducted by a veterinarian.

    VAN USE:

Our services are provided in the safety and comfort of our customized PET MOBILE salons. For liability reasons we do not allow customers within the vans, not do we allow more than one pet at a time in the van at any time. We will never leave a pet alone in the van, so we require a contact number to reach you during the appointment should we have any questions or concerns. We will require a place to park the van. By booking an appointment you are confirming there is a parking space available for the van. If we are unable to safely and legally park the van in order to provide services to your pet, the ‘no show’ fee above will apply.
